Overview

Bevacizumab (trade name Avastin) is a recombinant humanized monoclonal IgG1 antibody used as an anti-angiogenic therapy, chiefly in oncology. It binds vascular endothelial growth factor A (VEGFA) with high affinity and prevents it from engaging its receptors VEGFR-1 and VEGFR-2, blocking the signaling that drives new vessel formation in tumors. Because it neutralizes the ligand in the extracellular space, its mechanism differs from that of small-molecule tyrosine kinase inhibitors such as sorafenib, lenvatinib or cabozantinib, which act on receptor kinase domains inside the cell — a distinction that matters for toxicity, since the antibody hits one ligand while the kinase inhibitors hit many targets at once. Approved in 2004, it is given intravenously, generally in combination, across colorectal cancer, non-small cell lung cancer, glioblastoma, renal cell carcinoma, ovarian cancer and cervical cancer, paired with cytotoxic backbones such as irinotecan, carboplatin and paclitaxel, or cisplatin plus paclitaxel; in cervical cancer the alternative backbone is topotecan plus paclitaxel rather than fluorouracil-containing regimens. Anti-VEGF agents of this class, including Ranibizumab and aflibercept, are also given intravitreally in retinal vascular disease, where bevacizumab itself is widely used off-label at a fraction of the cost.

Beyond suppressing angiogenesis, VEGFA blockade remodels the tumor microenvironment. Vascular normalization can improve perfusion and drug delivery, while reduced VEGF signaling lessens the immunosuppression VEGF exerts on dendritic cells, regulatory T cells and myeloid populations — the rationale for combining it with checkpoint inhibitors. The combination with the PD-L1 antibody atezolizumab is now first-line standard of care for unresectable hepatocellular carcinoma, and analogous immunotherapy-plus-antiangiogenic strategies are under study with pembrolizumab and durvalumab.

The toxicities follow from blocking a pathway that healthy tissue also uses: hypertension and proteinuria from effects on endothelium and glomerulus, impaired wound healing that requires surgery to be spaced around dosing, arterial and venous thrombosis, bleeding — of particular concern in cirrhotic patients with portal hypertension — and, uncommonly but seriously, gastrointestinal perforation. Combined immune-targeted regimens add immune-related toxicities on top of these. Bevacizumab's capacity to reduce vascular permeability and edema has also prompted non-oncologic use, for instance in radiation-induced tissue injury.
